Care Coordination

The Care Coordination department assists patients and families throughout acute illnesses and recovery by assessing medical and psychosocial needs and facilitating access to appropriate levels of care and services.  In providing the above function, we ensure that patients are able to receive the maximum services and care within their defined insurance benefits.  The Care Coordination Department is staffed by Registered Nurse and Social Worker Case Managers.

The Care Coordination department maintains a comprehensive list of HUHS-preferred providers for Home Care Services. We welcome comments on any type of Home Care services our patients have received.
