The Care Coordination Department assists patients and families throughout acute illnesses and recovery by assessing medical and psychosocial needs and facilitating access to appropriate levels of care and services. In providing the above function, we ensure that patients are able to receive the maximum services and care within their defined insurance benefits. The Care Coordination Department is staffed by a Nurse Case Manager, a Social Work Case Manager and a Durable Medical Equipment Coordinator.

The HUHS Durable Medical Equipment (DME) Coordinator assists clinicians in making arrangements for medical equipment.
